服务对象：自然人。
服务内容：为我市持卡人员补、换社会保障卡。根据持卡人员的社保卡遗失、损坏等缘由，通过提取持卡人员身份信息再次制作社会保障卡。
申请条件：办理人属于重庆市社会保险参保人员，办理人社会保障卡损坏、到期或遗失，办理人需更换社会保障卡上相关个人信息。
申请材料：居民身份证（未成年人可用户口簿）。
文件依据：《关于印发“中华人民共和国社会保障卡”管理办法的通知》（人社部发〔2011〕47号）。
服务渠道：

Social security card replacement and renewal
Service recipient: Natural persons.
Service content: Replacement and renewal of social security cards for the residents of Chongqing. In case of lost or damaged social security cards, the card holder's identity information is extracted to make a new social security card.
Scope of application: The applicant has participated in the social insurance program of Chongqing; the applicant's social security card has been lost, damaged or expired; the applicant requires to change his/her personal information on the social security card.
Application materials: Resident ID card (household registration booklet if the applicant is a minor).
