服务对象：自然人。
服务内容：为我市新参保人员制作社会保障卡。通过经办网点收集的参保人员身份信息和系统参保信息进行比对，提取参保人员社保信息，结合银行信息加载到社会保障卡芯片内完成制卡。
申请条件：办理人属于重庆市社会保险参保人员，办理人首次办理社会保障卡。
申请材料：1.个人申领
（1）、居民身份证（未成年人可用户口簿）
（2）、一张1寸免冠登记照
2.批量申领
（1）、居民身份证（单位经办人）
（2）、《批量申领社会保障卡登记表》（含电子照片）

Service recipient: Natural persons.
Service content: Making of social security cards for newly insured persons in Chongqing. Compare the identity information of the insured persons collected by the handling outlets with the information stored in the computer system, extract the social security information of insured persons, and load it into the chip of the social security card together with the bank information to complete card making.
Scope of application: The applicant is the purchaser of social insurance in Chongqing Municipality and applies for a social security card for the first time.
Application materials: 1. Application by individual
(1) Resident ID card (household registration booklet if the applicant is a minor)
(2) One 1-inch bareheaded photo for registration
2. Bulk application
(1) Resident ID card (on behalf of a company)
(2) Registration Form of Social Security Cards Bulk Application (with electronic photos)
