申请条件：参保对象向社会保险经办机构查询和打印缴费和享受社会保险待遇记录。
申请材料：1. 本人居民身份证或社会保障卡；
2. 委托人和被委托人双方的居民身份证或社会保障卡。
依 据：《中华人民共和国社会保险法》。

Service recipient: Natural persons.
Service content: Social insurance agencies provide personal rights and interests record inquiry and printing services for insured persons in accordance with the prescribed scope.
Scope of application: Insured persons inquire and print the records of insurance premium payments and of enjoyed social insurance benefits via social insurance agencies.
Application materials: 1. Resident ID cards or social security cards of individuals;
2. Resident ID cards or social security cards of both the principal and the agent.
Legal basis: Social Insurance Law of the People's Republic of China.
