服务对象：法人、自然人。
服务内容：提供劳动能力初次鉴定申请服务。
申请条件：职工发生工伤，经治疗伤情相对稳定后存在残疾、影响劳动能力的，可向作出工伤认定的区县劳鉴办提出劳动能力初次鉴定申请。
申请材料：1. 《重庆市工伤职工劳动能力初次鉴定申请表》；
2. 有效病史资料（提供有效的诊断证明、按照医疗机构病历管理有关规定复印或者复制的检查、检验报告等完整病历材料）。

2. 《工伤保险条例》;
3. 《工伤职工劳动能力鉴定管理办法》;
4. 《重庆市人民政府关于印发重庆市工伤保险实施办法的通知》（渝府发〔2012〕22号）；

Application for Labor capacity appraisal
Service recipient: Natural persons and legal persons.
Service content: Provision of application services for initial appraisal of labor capacity.
Scope of application: If an employee is injured at work and loses his/her ability to work even after he/she is in stable condition by treatment, such employee may apply for the initial appraisal of labor capacity to the district- or county-level labor appraisal office that issues reports of employment injury appraisal.
Application materials: 1. Application Form for Initial Appraisal of Labor Capacity of Staff Members and Workers Injured at Work in Chongqing Municipality;
2. Valid medical history data (providing the valid proof of diagnosis, complete medical records such as examination and test reports copied or reproduced in accordance with the regulations on the medical record management of medical institutions).
